Position Qualifications:

  • Bachelor’s level degree in one of the recognized mental health professions preferred.
  • 4 years’ experience or training in the human service field in lieu of degree will be considered.
  • Requires successful completion of medication administration class within one month of hire.
  • Annual Flu immunization by Nov 1, annual TB screening, current CPI training and BLS certification required. 

Job Purpose:

Provide service coordination for clients receiving residential services within the ATU.  Participates as a member of the service delivery team to assist with the individual care plans for program clients in accordance with policies, rules, regulations and directives that apply.  Case management duties include assisting with benefits, housing, development of wrap plans specific to recovery issues, and assisting the client in building an independent support system.  Utilizes a variety of advocacy and service coordination techniques to work with a diverse client population in a residential setting.   May also be asked to provide similar services in other locations as needed. 

Knowledge, Skill & Ability

  • Ability to produce written documentation of clinical work in a manner readily understood by other service providers.
  • Ability to communicate verbally and in writing in a manner readily understood by others, and to effectively utilize a personal computer.
  • Ability to summarize service themes from a variety of client contacts and communicate suggested improvements for service delivery.
  • Ability to be flexible in carrying out diverse job duties.
  • Ability to provide an appropriate and responsible role model through actions and appearance.
  • Ability to deliver services based upon the consumer’s needs.  This may necessitate travel within the community.
  • Working knowledge of Windows based computer applications to include MS Word, MS Excel, MS Outlook, and Internet access.
  • Ability to operate standard office equipment including calculator, copy machine, fax and multi-line telephone.
  • Ability to organize and prioritize workload in a sometimes hectic environment with frequent interruptions.
  • Must have current driver’s license and ability to be commercially insured and maintain a driving record that allows insurability with AHS commercial insurance agency.
